Alabama enforces a total abortion ban with narrow life or health exceptions, creating one of the most restrictive legal environments in the country. Maternal mortality is among the nation’s highest, with stark racial disparities that disproportionately affect Black mothers. The state has not expanded Medicaid under the ACA, and many rural counties are maternity-care deserts with few or no obstetric providers. Hospital closures and long drives to prenatal and delivery services compound risk, especially for high-risk pregnancies. As a result, legal prohibition and thin healthcare infrastructure jointly limit timely, comprehensive pregnancy care.
